I understand that participating in a martial arts event is considered a physical activity. I understand and acknowledge the risks that are involved in any such physical activity. I hereby release all claims against the guests, members, owners, and staff of Competitive Edge Mixed Martial Arts. I give my child permission to participate in all functions and activities held at the school or any classes held off site.

I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and consent to participate in this program. I agree to disclose my physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program.
